Success Coach Service (SCS)
A free, voluntary, home-based program designed to proactively strengthen families by:
- Increasing protective factors
- Reducing risk factors
- Preventing out-of-home placement
- Supporting long-term family stabilization
The program uses an evidence-based approach focused on individualized goal setting, family engagement, and coordinated wraparound services.
Who We Serve
Families in Forsyth County with teens 11-18 years old, particularly those involved with or at risk of involvement in child-serving systems such as:
- Child Protective Services (CPS)
- Juvenile Justice
- Winston-Salem Police Department
- Temporary foster care placements
- "Safety Hold" children at Brenner's Pediatric Emergency Room
- Behavioral or mental health services where caregivers need system navigation support to prevent out-of-home placement
Program Structure
- Duration: Up to 2 years of voluntary participation
- Service Delivery: In-home visits and case management
- Focus areas include:
- Parenting skills and family communication
- Youth behavior and supervision
- Crisis planning and safety
- Connection to community and mental health resources
- System navigation and advocacy
Upon successful completion, families may access Booster Sessions for up to 3 additional months to address new or emerging concerns.

Referral Information
All referrals must be made through Forsyth County Department of Social Services (DSS) and are offered free to eligible families.
